MyCare Plan™ will provide each purchaser with the following:

• Last Will
• Living Trust
• Financial Power of Attorney
• Health Care Power of Attorney

All document completion services will be performed on behalf of LawMart.com, Inc. by its exclusive agent, Financial Education Services, Inc., a federal credit union service organization.

After ordering a MyCare Plan will and trust package, the purchaser will be responsible to complete a data input document that will be used to create the will and living trust. There is no time limit on completing the data input document; however, the will and trust cannot be produced without the necessary information.

The purchaser may cancel this transaction within three (3) business days after purchase and receive a full refund. After three (3) business days, no refund will be issued.

If a dispute arises out of this transaction, and if it cannot be settled by the parties, the parties agree first to try to settle the dispute by mediation administered by the American Arbitration Association ("AAA"), under its Commercial Mediation Procedures. If they do not resolve the dispute within 60 days, then, upon notice by either party to the other, all disputes, claims, questions or differences shall be finally settled by binding unappealable arbitration administered by the AAA. In any such proceeding, which must take place in the State of Michigan, each party shall bear their own attorney fees and costs, the fees and costs of any arbitrator selected by them, and one half the fees and costs of any neutral mediator or neutral arbitrator.

Once the purchaser has given final approval of the input data, any changes requested will be subject to a $99.00 fee.
